What this site collects

When you send the consultation form we receive your name, phone number, email address, the treatment you chose and the times you gave us. That's all the form asks for, and we ask you not to include medical information in it.

The site records anonymous page views and whether a consultation request was sent. It does not use advertising cookies and does not build a profile of you.

What we do with it

Form submissions go to our booking inbox and are used to contact you about a consultation. We don't sell or share them and we don't add you to a mailing list unless you ask to be on one.

Your medical record, once you're a patient, is kept in our practice management system under HIPAA and New York State law. It isn't connected to this website.

Removing your details

Email hello@moonshinemedspa.com from the address you gave us and we'll delete your form submission within ten business days and confirm when it's done.
